UV เรื่องไม่เล็กที่ควรรู้ ทุกวันนี้โลกร้อนขึ้นทุกวัน แล้วแดดก็แรง ที่เป็นเช่นนั้นเพราะว่าชั้นบรรยากาศของโลกถูกทำลายมากขึ้นทุกวัน ชั้นบรรยากาศที่ป้องกันแสง “ยูวี” ที่ชื่อ Stratospheric Ozone ถูกทำลาย ซึ่งมีอยู่ประมาณ 90 % ที่ระดับความสูงประมาณ 25 กิโลเมตร ซึ่งอยู่ในชั้นบรรยากาศสตราโตสเฟียร์ (ระยะความสูงจากพื้นดิน 17-50 กิโลเมตร) โอโซนชั้นนี้จัดว่าเป็นโอโซนที่ดี ช่วยปกป้องแสง UV จากดวงอาทิตย์ไม่ให้ตกกระทบลงบนพื้นผิวโลกมากเกินไป แสงจากดวงอาทิตย์ที่สาดส่องมาถึงโลกนั้นประกอบด้วยรังสี 2 ประเภทคือ ประเภทที่ตามองเห็นได้กับประเภทที่ตามองไม่เห็น สำหรับแสงหรือรังสีชนิดที่ตามองเห็นได้ก็คือ “แสงสว่าง” นั่นเอง แสงประเภทนี้เรารู้จักกันดี เพราะว่าช่วยให้เรามองเห็นโลกที่สวยงามใบนี้อยู่ทุกวันตั้งแต่เช้าจรดเย็น แสงสว่างนี้ถ้าเราปล่อยให้วิ่งผ่านปริซึม ก็จะเห็นว่าประกอบด้วยแสงสีรุ้งทั้ง 7 สีคือ ม่วง คราม น้ำเงิน เขียว เหลือง แสด แดง แสงสว่างเหล่านี้มีพลังงานไม่สูงจึงไม่มีพิษภัยต่อร่างกายมากนัก แสงมืด แสงที่ตามองไม่เห็นนั้นประกอบด้วยรังสี 2 ประเภทคือ แสงอินฟราเรด (Infrared) หรือที่รู้จักกันดีในรูปของ “ไอร้อน” และแสงอัลตร้าไวโอเล็ต (Ultraviolet) หรือที่เรามักได้ยินว่าแสง “ยูวี” อินฟราเรดที่ ไม่เป็นอันตรายต่อร่างกาย(แสงอินฟราเรดนี้พบได้ในไอแดด,ไอร้อนจากเตา,น้ำ ร้อน,กระทั่งไออุ่นจากร่างกายคน) แต่มีพลังงานสูงพอที่จะใช้รักษาโรคผิวหนังได้หลายชนิด เช่น ไฝ กระ หูด รอยสัก เนื้องอก และมะเร็งผิวหนัง เป็นต้น แสงยูวีเป็นแสงที่มองไม่เห็นแต่มีพลังงานสูงมาก แสงยูวีนี้แหละที่เป็นสาเหตุทำให้เกิดมะเร็ง และเนื้องอกมากมายหลายชนิด แสงในความยาวคลื่นของอุลตราไวโอเลต(UVL=Ultraviolet light) แบ่งเป็น ultraviolet A(UVA wavelength 320-400nm), ultraviolet B( UVB wavelength 280-320nm) และ ultraviolet C(UVC wavelength 190-280 nm) เฉพาะUVA และ UVB(>290nm) เท่านั้นที่ส่องมาถึงพื้นโลก ส่วน UVC ซึ่งเป็นอันตรายอย่างมากต่อนิวเคลียสของเซลล์ จะถูกกรองโดย โอโซนในบรรยากาศชั้น stratosphere ทำให้ไม่สามารถผ่านมาถึงพื้นโลก UVB จะผ่านเข้าไปในผิวหนังได้แค่ชั้นหนังกำพร้า และชั้นตื้นๆของหนังแท้ เป็นตัวการสำคัญที่ทำให้เกิดการไหม้แดด (sunburn) ถ้าได้รับในปริมาณที่มากเกินไป และถ้าได้รับมากในระยะยาว อาจก่อให้เกิดมะเร็งผิวหนังได้ UVA ผ่านผิวหนังได้ลึกกว่า คือลงไปถึงชั้นหนังแท้ ซึ่งถ้าได้รับมากเกินไป จะไปทำลายเนื้อเยื่อคอลลาเจน และอิลาสติก ทำให้เกิดการแก่ก่อนวัย (photoaging) ปัจจุบันเชื่อว่า UVA ก็เป็นสาเหตุของมะเร็งผิวหนังด้วยเช่นกัน ตัวการสำคัญที่สุดก็คือส่วนที่เป็นแสงยูวีหรืออัลตร้าไวโอเล็ต ซึ่งเป็นรังสีที่มาพร้อมกับแสงแดดนั่นเอง แสงยูวีโดยเฉพาะอย่างยิ่งยูวีเอนั้นเป็นตัวการร้ายที่สุด จะเรียกว่ามหันตภัยมืดก็ได้ เพราะว่าเป็นแสงที่ตามองไม่เห็น (ไม่เกี่ยวกับว่าแดดจ้าหรือไม่) ไม่ร้อน ไม่แสบ และสามารถทะลุผ่านกระจกได้ด้วย การป้องกันแดดที่ถูกวิธีนั้น เราต้องมีทัศนคติที่ถูกต้อง และการปฏิบัติตัวที่ถูกวิธีซึ่งพอสรุปเป็นข้อๆ ได้ดังนี้ 1. การถูกแสงแดดไม่ว่ามากหรือน้อยเพียงใดก็ตาม ต่างก็เป็นอันตรายทั้งสิ้น ทั้งนี้เนื่องจากพิษของแสงแดดจะสะสมไปเรื่อยๆเหมือนกับการออมทรัพย์ ต่างกันแต่เพียงว่าธรรมชาติมีความซื่อสัตย์ยิ่งกว่ามนุษย์ ดังนั้นเมื่อสะสมไปเป็นเวลานาน ก็จะได้รับพิษทั้งต้นและดอกเบี้ยครบด้วยบริบูรณ์ไม่มีการโกงครับ 2. แสงแดดไม่ว่าเวลาใดก็ตามมีอันตรายทั้งสิ้น ผมเชื่อว่าพวกเรา หลายๆ คนคงจะเคยได้ยินจากหมอหรือได้อ่านจากหนังสือ ว่าให้หลีกเลี่ยงเฉพาะแสงแดดช่วงตั้งแต่ 9 โมงเช้าถึงบ่าย 3 โมง ความเชื่อดังกล่าวเป็นสิ่งที่ผิดพลาดอย่างร้ายแรง ทั้งนี้เนื่องจากว่าในความเป็นจริงแล้วแสงยูวีชนิดเอ (UVA) จะมีปริมาณค่อนข้างคงที่ตลอดทั้งวัน ในขณะที่แสงยูวีบีจะมีปริมาณมากเฉพาะในช่วง 9 – 12 นาฬิกาดังกล่าว จริงอยู่ว่าพิษจากยูวีบีนั้นแรงกว่ายูวีเอ แต่ก็อย่างที่เคยได้อธิบายให้ฟังแล้วว่า ยูวีบีนั้นถูกเพียงไม่กี่ นาทีก็แสบแล้ว ดังนั้นคนส่วนใหญ่มักจะวิ่งหนีไปก่อนที่จะได้รับพิษมากพอ นอกจากนี้ยูวีบียังทะลุกระจกไม่ได้ (ถ้ามีความหนาเกิน 3 มม.) ดังนั้นเวลานั่งอยู่ในห้องหรือในรถยนต์ก็มักจะปลอดภัยจากแสงนี้ ขณะที่ยูวีเอทะลุมาได้เฉยๆ เลย ทางที่ดีที่สุด ทุกคนจึงควรหลบแดดไม่ว่าเวลาใดก็ตาม แต่สำหรับท่านที่ผิวยังดีอยู่คือ ไม่มีรอยย่นหรือกระ ฝ้า ก็พอจะถูกแดดช่วงเช้าๆหรือเย็นๆได้แต่ถ้าไมุถูกเลยจะดีที่สุด ส่วนท่านที่มีกระ มีฝ้า หรือหน้าย่นอยู่แล้ว ก็เปรียบเหมือนกับคนที่ดื่มเหล้าจนตับแข็งแล้ว แม้จะดื่มอีกเพียงเล็กน้อยร่างกายก็รับไม่ได้ 3. การป้องกันแดดที่ดีที่สุดคือการหลบแดด ไม่ใช่การทาครีมกันแดด หรือมาตรการใดๆ ก็ตาม ขอให้จำไว้ว่าไม่มีวิธีการอะไรที่จะสามารถทดแทนการหลบแดดได้ ทากันแดดหรืออะไรก็ตาม วิธีการเหล่านั้นเป็นเพียงมาตรการเสริมเท่านั้น ซึ่งก็เป็นประโยชน์จริงแต่ก็ไม่มีวิธีใดที่แทนการหลบแดดได้ 4. ถ้าจำเป็นต้องออกไปถูกแดด ก็ขอให้พยายามหาวิธีผ่อนหนักเป็นเบา เช่น เลือกไปออกแดดเฉพาะเวลาที่แดดอ่อนๆ (เช้าๆ หรือเย็นๆ) ถ้าถือร่มได้ต้องใช้”ร่ม“สี เงินหรือตะกั่ว ซึ่งจะสะท้อนแสงแดดออกไปได้มากที่สุด ทายากันแดดทุกวัน และพยายามสวมใส่เสื้อผ้าที่ช่วยปิดบังมิดชิดที่สุดเท่าที่จะทำได้ ถ้าสวมหมวกได้ก็ควรสวมไว้ แว่นดำก็จะช่วยป้องกันการเกิดต้อกระจกได้ สำหรับใบหน้านั้น ถ้าไม่อายใครก็ขอให้เอาผ้าพันปิดไว้เลยเหมือนอย่างชาวอาหรับหรือชาวไร่ชาวนา เขาทำกัน อย่าไปดูถูดวิธีง่ายๆอย่างนี้นะครับ ผิวเขาสวยขนาดประกวดได้เลย โดยไม่ต้องใช้ของแพงๆเหมือนพวกเรา สุดท้ายสำหรับมือก็ควรสวมถุงมือไว้ด้วยจะได้ป้องกันไม่ให้หลังมือเหี่ยวย่น นัก 5. การใช้ครีมกันแดดที่ถูกต้องนั้นควรเลือกครีมกันแดดชนิดที่ป้องกันได้ทั้งแสง ยูวี เอและยูวีบี เลือก ใช้ชนิดที่มีเบอร์เอสพีเอฟ (SPF = Sunburn Protection Factor) สูงๆ ตั้งแต่ 15 – 30 ขึ้นไป เพื่อให้ป้องกันแสงยูวีบีได้นานที่สุด ส่วนประสิทธิภาพของเนื้อยาที่ช่วยป้องกันแสงยูวีเอนั้น ยังไม่มีวิธีวัดที่ดีดังนั้นเวลาเลือกซื้อก็คงได้แต่ดูว่ามีสารป้องกันยูวี เอหรือไม่ยิ่งมีหลายชนิดยิ่งดี การทาครีมกันแดดที่ถูกวิธีนั้น ต้องทาก่อนออกแดดอย่างน้อย 30 นาที ทาทุกวันไม่ว่าจะเป็นวันที่แดด มามากหรือมาน้อยก็ตาม และประการสุดท้ายต้องทาไว้หนาพอ หมายความว่าถ้าใช้อย่างครีมควรจะทาครีมไว้จนเห็นคล้ายกับทาแป้งไว้บางๆ ถ้าทาแล้วมองไม่ออกเลยว่าทาอะไรอยู่แปลว่าทาน้อยเกินไป จะไม่ได้ผลอย่างที่คาดหวังไว้ ความจริงแล้วทายิ่งหนายิ่งดี ยิ่งถ้าทายาจนมองไม่เห็นผิว เหมือนกับว่าเอาแป้งมาพอกไว้ตอนเล่นงิ้วละก็ดีที่สุด 6. หลีกเลี่ยงการไปท่องเที่ยวริมทะเล เนื่องจากน้ำทะเลและหาดทราย สามารถสะท้อนแสงแดดขึ้นมาในแนวราบได้ 30 – 100 เปอร์เซ็นต์ ขึ้นอยู่กับมุมตกกระทบของแสง มีคนไข้จำนวนมากที่ไปเผลอหลับอยู่ใต้ร่มริมทะเล พอตื่นขึ้นมาหน้าทั้งแดงทั้งบวมอย่างกับไปถูกใครต้มหรือลวกเอา คนไข้เองก็งง เพราะว่าไม่ได้ไปนอนกลางแดด แต่ถ้าเราเข้าใจว่าหาดทรายและสายน้ำรอบข้างนั่นแหละทำตัวเหมือนกระจกสะท้อน แสงมาก็คงจะหายงง 7. กิจกรรมใดๆที่ไม่จำเป็นต้องเกี่ยวข้องกับแสงแดด พยายามเลือก ประกอบกิจกรรมนั้นๆในช่วงที่มีแดดน้อยที่สุด ยกตัวอย่างเช่น การว่ายน้ำเป็นการออกกำลังกายที่ดีที่สุด แต่ไม่จำเป็นต้องไปว่ายกลางแดดเวลาเที่ยงหรือบ่ายๆ พยายามว่ายกลางคืนจะดีที่สุด การไปตรวจงานก็เช่นกัน ถ้าไปตรวจแต่เช้ามืดหรือเย็นๆ ได้ก็จะดี อย่าไปตรวจเวลาเที่ยงโดยเด็ดขาด เพราะว่าปริมาณแสงยูวีช่วงเที่ยงวันนั้นมีความแรงรวมมากกว่าช่วงเช้าเป็นพัน เท่าเลยทีเดียว มาถึงตอนนี้ก็อย่าเพิ่งหมดแรงอ่อนใจเลยครับ ผมไม่ได้คิดว่าพวกเราจะทำได้ครบถ้วนร้อยเปอร์เซ็นต์เหมือนการถือศีลละครับ ถือได้บางข้อก็ยังดีกว่าไม่ได้เลย สิ่งที่สำคัญที่สุดก็คือ เราต้องรู้ว่าทุกๆ อณูของแสงแดดนั้นเป็นพิษ ยิ่งเราหลีกเลี่ยงได้มากเท่าไร ก็จะเป็นบุญแก่ผิวพรรณและร่างกายเรามากเท่านั้น ประการสุดท้ายที่สำคัญก็คือ ควรหลบตั้งแต่เล็ก ตั้งแต่แรกเกิดเลยยิ่งดี ไม่ใช่รอจนทั้งเหี่ยวทั้งดำแล้วจึงมากระมิดกระเมี้ยนหลบแดดนะครับ พอจะได้ความรู้กันพอสมควร ง่ายคือการหลบแดด ถ้าจำเป็นก็หาร่มหรือใส่เสื้อผ้าหนาๆ ทึบแสง อะไรที่ทึบแสงก็กันแสงแดดได้มากพอสมควรแล้วล่ะครับ
